Terms and Conditions for cosmetic injections:
– Bookings must be made in advance.
– Cosmetic injection clients must be deemed medically fit and suitable for treatment, following a consultation.
– Only the 30-minute consultations are complimentary.
– Price per area is defined upon consultation as a sufficient product to be used to achieve the medically appropriate results.
Terms and Conditions – Before and After Campaign
SDR Cosmetic Injectables would like to gather resources such as photographic images, written testimonials, video, or audio from you to create educational and promotional material. These images and resources will contain information about your medical diagnosis and treatment. As such, it would be regarded as sensitive data, which by law requires your explicit consent to allow us to process it. If chosen to participate, you will be required to sign a legal consent form by which you will confirm that the person named in the release notice has agreed to the publication of the resources by SDR Cosmetic Injectables both now and in the future. Through the signed consent, SDR Cosmetic Injectables will be granted the right to use the resources on such materials, as it deems appropriate. This includes and is not limited to websites, the Internet, newsletters, publications, radio, television and newspapers etc. In doing so,SDR Cosmetic Injectables   may crop and/or college any of the resources. By signing the consent form, you release and forever discharge SDR Cosmetic Injectables, its agents, officers and employees, from any claims and demands arising out of or in connection with the use of the photographic images, written testimonials, video and or audio, including but not limited to, all claims for invasion of privacy, defamation, or financial compensation. You waive all moral rights that you may have in the photographic images, written testimonials, video, or audio. 
 CANCELLATIONS & RESCHEDULING

Your appointment is important to us. They are reserved especially for you and we are committed to providing all patients with exceptional service. When a patient cancels without allowing enough notice, they prevent another patient from having an appointment. Booking Fees are non-refundable and non-transferable upon making your appointment.

·       We understand that sometimes you may need to change your appointment, but we ask that you give us at least 48 hours’ notice prior to your appointment time so that we can offer your time slot to another client.  

·        Last minute cancellations and rescheduling of your appointment, less than 24 hours’ notice (during our business hours) will result in a cancellation fee of deposit paid. Packages and Gift Certificates are non-refundable.

·       Failing to not show up for an appointment will incur the fee of 50% of your treatment cost for that day. Packages and Gift Certificates are non-refundable.

 medical consult booking fee of $50 is required for appointments and is non-refundable and non-transferable. This will be deducted from treatment on the day. If you cancel your appointment  in less than 24 hours and require and new appointment, a new booking fee of $50 will be required.

If you are unable to attend your scheduled appointment, within the notice period please send a friend or family member in your place and we
will happily accommodate them.

As a courtesy we send out text message reminders of your scheduled appointment day and time, however please do not rely on these “reminders”, always add appointments into your calendar, this is your responsibility.

If you do not accept our cancellation policy we welcome you booking your appointment on the day you would like to come in. However, please be mindful that you may not get an appointment. We hope that you will appreciate our Cancellation policy.

misplaced or stolen.

Arrival: We request that you arrive at least 5-10 minutes prior to your scheduled appointment time in order to ensure you have time to complete any necessary paperwork and relax. We regret that late arrivals will receive condensed services. If you arrive more than 10 minutes later than the scheduled start time, we will have to rebook you for another day with a new booking fee required for that appointment. Please be mindful of arriving late as it takes away from your treatment and you cannot be guaranteed all your requests can be completed on the day.

Medical Conditions: If you have any type of medical condition or allergies please be sure to mention it when you are booking your appointment. Certain treatments may not be advisable for you. Also, before your treatment begins, let your registered nurse know of any medical concerns.

Please advise us at the time of your appointment of any health conditions, allergies, injuries, recent or upcoming surgeries, pregnancy or special needs which may affect your services. 

Patients cannot have Cosmetic Injections:

-COVID
vaccine; allow 2 weeks prior and after anti-wrinkle & dermal filler
procedures.
-Sick/unwell
-Having Chemotherapy
-Pregnant/Breastfeeding or trying to conceive, (IVF)
-Active cold sores (if you are prone to cold sores, please take over the
counter Anti-viral tablets 3 days prior to appointment)
-Open wounds to area being treated
-Flying within 14 days of dermal filler appointments
-Upcoming medical procedures (please inform your medical practitioner)
-Allergies to Bees or Wasps.

 Children: While we love children we ask that you please make other arrangements for them for your time at the clinic. Due to health and occupational hazards we are not insured to have children within the premises left unattended. 

Mobile Phones: To maintain a relaxing environment and avoid annoying those around you, we ask that all mobile  phones be turned off/silent mode  while visiting the clinic.

 COVID

 

If you are experiencing flu/cold like symptoms please cancel/reschedule your appointment by texting SDR Cosmetic Injectables on 0422716437. If its within 24 hours of your appointment, you will forfeit the booking fee.

If your appointment cannot go ahead due to COVID restrictions the booking fee will be kept as a credit on your file. If you need to cancel last minute due to waiting for Covid results you forfeit the booking fee.

If you are feeling unwell, or have any COVID symptoms (dry cough, unwell, fever, muscle aches) please let us know so we can reschedule
your appointment. Having a medical procedure is not recommended and can have impacts on your treatment and health. We also want to ensure you that SDR Cosmetics Injectables  is an open space, and want to make sure to respect and protect everyone’s well-being.

REFUNDS:

 

Products used for non-invasive treatments are a Scheduled 4 Drug
and legally can not be refunded upon signing the pre- procedure consent forms
on the day of procedure.
All cosmetic procedures are non refundable, your clinician may
offer discounts, complimentary services touch ups or dissolving at their
digression.

We accept all major credit and debit cards. Afterpay available.

Cash is also accepted.
